On this particular flight, the two parachute systems (one for the booster, one for the camera payload) got twisted together. Also, the D12-5 motor ejected itself as well as the parachutes, and took the metal engine clip with it!  The camera window can be seen in the silver section of the payload, the black square is facing just to the right.
